Worth saying plainly: the ASRM's 2021 guideline on oocyte cryopreservation concluded there is insufficient evidence to counsel women of any given age on the absolute number of eggs needed for a reasonable chance of a live birth, and insufficient data to name an optimal age to freeze at all. Its own illustrative figures put roughly 14 mature eggs at ages 30–34 and 26 at ages 38–40 for a 70% chance. Thirty-seven sits between those, which is exactly where the evidence is thinnest.

What happened to women who actually froze at about 37

Model estimates and outcomes are different things, and at this age the real-world record is unusually relevant — because 37 is close to the average age at which women actually do this. The 2024 systematic review by Hirsch and colleagues in Human Reproduction Update pooled 10 studies and 8,750 women whose mean age at freezing was 37.2. Among them, 11.1% ever returned to use their eggs. Of those who did, 78.5% of eggs survived warming and 28% had a live birth. Split by age at freezing, women who froze at 35 or younger reached 52%; those who froze at 40 or older, 19%.

Cobo's 2018 cohort shows the same fault line at a different cut point: among elective patients who returned, live birth per patient was 68.8% for those who froze at 35 or younger and 25.5% for those who froze after 35. And in a 15-year single-center series of 543 patients at NYU Langone (Cascante and colleagues, Fertility and Sterility 2022), the final live birth rate was 39% per patient overall, but above 50% for two specific groups: women under 38 at the time of freezing, and women who thawed at least 20 mature eggs.

That last finding is the most useful sentence on this page. At 37 you still clear both of those bars — but only just, and only if you bank the eggs. Next year you clear one.

The per-cycle price does not change with age; the number of cycles does. US clinics commonly quote $12,000 to $20,000 all-in per cycle including medication, plus $500 to $1,000 a year in storage. Decisions made at 35 could be sloppy and still work. At 37 they need to be deliberate: a real egg target, a realistic cycle count, a clinic whose own thaw outcomes you have seen.
